What Is Assessment & Evaluation?

Assessment and Evaluation is the foundation of effective therapy. Our occupational therapists conduct comprehensive assessments to identify your strengths, challenges, and functional goals — giving us the clear picture needed to plan the right intervention from the start. Assessments are standardised, evidence-based, and tailored to your age and presentation.

What We Assess

Types of Assessment We Offer

We offer a broad range of assessments across functional, cognitive, sensory, and environmental domains — covering all areas that impact daily life.

Functional Capacity

Evaluates your ability to perform everyday activities including self-care, mobility, household tasks, and community participation — often required for NDIS planning.

Sensory Processing

Identifies how the nervous system processes sensory input and how this affects behaviour, attention, and daily functioning — informing therapy and environmental strategies.

Cognitive & Executive Function

Assesses memory, attention, planning, and problem-solving skills — commonly used following acquired brain injury, stroke, or for individuals with ADHD or intellectual disability.

Home & Environmental

Evaluates the safety, accessibility, and suitability of your home environment — forming the basis for home modification recommendations.

Assistive Technology

Determines the most appropriate assistive devices and equipment to support independence — required for NDIS mid-to-high cost AT funding approval.

Initial Consultation

We discuss your referral, background history, and current concerns to determine which assessments are most appropriate for your situation and goals.

Standardised Assessment

Using validated, evidence-based tools, we assess the relevant functional areas. This may include clinical observation, structured tasks, questionnaires, and home or community visits.

Goal Setting

Together with you, your family, and any relevant support team members, we identify meaningful therapy goals based on assessment findings and your priorities.

Report & Recommendations

A comprehensive written report is provided — summarising findings, goals, and recommendations. Reports meet NDIS documentation standards and can be shared with your support coordinator, GP, or school.

Frequently Asked Questions

Common questions about our services, appointments and insurance

What funding can I use to pay for support?
- We currently accept either NDIS funding or private payments for support. Ignite Therapeutic Solutions accepts plan managed and self-managed NDIS funds only.
What is the session duration?
- Initial Assessment: We will begin with an initial assessment to understand your needs, goals, and challenges. This assessment will help us develop a personalised therapy plan.
- Treatment Plan: Based on your assessment, we will create a treatment plan with specific goals, frequency, and methods of therapy. We will review and adjust this plan as needed throughout your therapy.
- Session Duration and Frequency: Therapy sessions typically last 60 minutes, this includes 10-15 minutes of clinical notes completion. The frequency of sessions will be discussed and agreed upon during the initial assessment and may change as your needs evolve. Initial and follow up Assessments for Functional Capacity Assessment typically last 90 minutes. AT trials typically last between 60 to 90 minutes depending on type and number of AT items being trialled
Where are the sessions held?
Services may be provided at your home, or other agreed-upon locations. We provide home/community based services to Perth Metropolitan Area. We can also provide Telehealth services to clients across Australia and to rural/remote areas.
What are the Transport Costs
Ignite Therapeutic Solutions charges for Provider Travel Time and Kilometres in line with the NDIA Price Guide found at www.ndis.gov.au/providers/price-guides-and-pricing.
What is the cancellation policy?
We understand that sometimes unforeseen circumstances may require you to change or cancel an appointment. If this happens, please contact us at 0459944487 or via email at fari.osawe@ignitets.org at least 2 working days (48 hours) in advance. Since we schedule our appointments ahead of time, we will need to charge the full cost of the appointment if we don’t receive cancellation notice within 2 days. However, no charge will apply if we are notified more than 2 days in advance. If we need to change or cancel an appointment, we will inform you as soon as possible and offer alternative options.
Privacy and confidentiality
Your privacy is important to us. All information shared during your therapy sessions, including medical history, assessments, and treatment plans, will be kept confidential, unless required by law or with your explicit consent. We will discuss any disclosures with you prior to releasing information to others.
The collection, holding, use and disclosure of personal information by Ignite Therapeutic Solutions PTY LTD is protected by the Privacy Act 1988 (Cth) (Privacy Act) and the National Disability Insurance Scheme Act 2013 (Cth) (NDIS Act).
We hold personal information which is reasonably necessary for us to carry out our role. Examples of information Ignite Therapeutic Solutions PTY LTD may hold includes:
- Name, contact details date of birth and age
- Gender, details about participants’ physical/ mental health, including disabilities
- Information about participants’ support requirements
- Details of guardians/ nominees, including names, addresses and contact details
- Details of feedback or complaints about services provided by us
- Bank account details
Personal information is securely destroyed, deleted, or archived once no longer required, in compliance with applicable federal privacy laws and regulations.
